Job Description
Will be responsible for assisting adults, children and families to connect with resources, support them upon discharge from higher levels of service and/or to bridge to longer term services for continued stabilization. Will be responsible for providing community outreach through various community-based events and collaborating/conducting presentations with local schools and community partners.
